Insurance Information:
Services may be covered in full or in part by your health insurance plan. To inquire further regarding your mental health benefits, please call the member services number listed on the back of your insurance card. Clients are responsible for confirming benefit coverage prior to the first appointment. We recommend you ask your insurance the following questions:

Payment:
To prevent disruption of therapy to discuss finances, we require all clients keep a credit or debit card on file as well as a signed consent for automatic billing.
Your card is stored in our Secure Electronic Health Record System.
Major credit cards, including HSA and FSA, are accepted.
Checks are also accepted for payment.

We do not bill insurance plans that are out-of-network for our providers.
Self Pay:
Self pay is available for clients not wanting to use their health insurance benefits. Sliding scale is available based on family income. Please contact the practice for more information.
Cancellation Policy:
When a client does not attend a scheduled session, we are not able to bill insurance, therefore we charge a $115 fee for no shows or sessions that are cancelled with less than 48 hours notice (subject to change).
Current clients should message their provider via their Therapy Portal as soon as they know they will need to miss/reschedule a session.

~Good Faith Estimate~
For Uninsured and Self-Pay clients:
You have the right to receive a 'Good Faith Estimate' explaining how much your mental health care will cost. Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
You can ask Lake City Counseling for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
