Insurance Information:

Services may be covered in full or in part by your health insurance plan. To inquire further regarding your mental health benefits, please call the member services number listed on the back of your insurance card. Clients are responsible for confirming benefit coverage prior to the first appointment. We recommend you ask your insurance the following questions:

• Do I have mental health insurance benefits?

• What are my out-of-network benefits?

• What is my deductible and has it been met?

• How many sessions per year does my health insurance cover?

• What is the coverage amount per therapy session?

• Will I owe a copay or coinsurance for each session?

• Is approval required from my primary care physician?

Payment:

To prevent disruption of therapy to discuss finances, we require all clients keep a credit or debit card on file as well as a signed consent for automatic billing. Client balances will be charged to the card on file on or around the 15th of the month.

Your card is stored in our Secure Electronic Health Record System.

 Major credit cards, including HSA and FSA, are accepted.

Checks are also accepted for payment.
